Caregiver handoff checklist: make the next shift easier
Organize contacts, routine responsibilities, current instruction locations and unresolved questions for a family caregiver handoff.
Hand over the current plan, not a memory of it #
A practical caregiver handoff says who is responsible, where current instructions live and what needs attention next. Confirm the person’s preferences about sharing information. This sheet should refer to instructions supplied by the care team rather than create new medication or treatment directions.
Cover the next period of care #
Specify the dates or times covered by the handoff. Separate routine responsibilities from questions that need a professional answer.
- Person coordinating the handoff and their backup.
- Usual provider and other appropriate contact routes.
- Location and date of the current care instructions.
- Appointments, transport, meals and other agreed daily responsibilities.
- Tasks completed, unresolved questions and the next check-in.
Use a simple status format #
A short update is easier to act on than a long chat history. Avoid unexplained shorthand, and distinguish observations from decisions or clinician instructions.

Review instructions after a hospital stay #
A discharge can change the information everyone has been using. AHRQ’s family-engagement guidance emphasizes understanding the next steps and asking questions before leaving. Point helpers to the current written plan and refer uncertainty back to the care team.
Ask the receiving helper to explain the next step #
Have them identify the current instruction location, the contact for questions and the first responsibility they are taking over. Correct anything unclear before the handoff is complete. Update the sheet when responsibilities change and remove stale copies from everyday use.
